for filling out a claim form

When you
receive your claim form in the mail, you will need to do the
following:
- Write your
Social Security number on the line provided for the Payee’s SSN
located in the middle of the claim form.
If you
are claiming property on behalf of a business:
- Enter the
business federal Employer Identification Number (EIN)
on the line provided for the
claimants's EIN, and
- Print and
complete the Business
Affidavit on your business letterhead.
- Sign and date the
form at the bottom on
the line marked "Your Signature."
- Write your daytime
phone number on the line provided for the
Claimant’s
Daytime Telephone Number, which is located directly
underneath the signature line.
- After the question asking how you
found your name on Idaho's unclaimed property list, check
one of the boxes.
- Enclose a photocopy of your driver’s license or some other type of
personal identification, which includes a photo and a
signature, such as a state issued ID
card, military ID
card, or passport. This is required even if you are
claiming property on behalf of a business.
- Mail your claim
form and documentation in an envelope addressed to:
