The information described below is for use in filing an unclaimed property report in the state of Idaho.  If you have never filed a report with Idaho, please review each item below before completing your report.

UP-1  (Idaho Report of Unclaimed Property):
This form is used as the transmittal document and payment voucher for unclaimed property.

UP-2   (Detail Reporting of Unclaimed Property):
This form is used to report detailed information about property to be delivered and to identify information about the owner of the property.
